You MUST:   Print the form.   Sign the form.   Send your printed, signed form via postal or courier service. If we do not receive your completed, signed Submission & Authorization Form, your work cannot be accepted. If you do not have Web access, contact the program chair at least two weeks prior to the deadline in order to make alternate arrangements.
